Purtscher's Retinopathy: Visual Outcome and Differential Diagnosis

Purpose: Severe visual loss after trauma of the thorax or the skull occurs in Purtscher's retinopathy (yellowish-white, polygonal spots in the retina, cotton-wool spots, flame-shaped or spot-like hemorrhages). Visual outcome is not consistent according to the literature.
Patients: Follow-up of ten patients (age: 21-65 years) with classical signs of Purtscher's Retinopathy after a traffic accident.
Results: The mean visual acuity after trauma was 0.1 (hand motions up to 0.4). The mean final visual acuity after several months was 0.6 (0.1 up to 0.9). Differential diagnosis: In three further patients atypical changes of the fundus were found after polytrauma: central retinal artery occlusion, traumatic optic atrophy, traumatic macular edema. They were primarily misdiagnosed as Purtscher's retinopathy although not showing the typical signs of the fundus.
Conclusions: In contrast to the initial severe retinal changes in Purtscher's retinopathy visual outcome often is favourable.
