The Photopic Negative Response of the Blue-on-Yellow Flash-Electroretinogram in Glaucomas and Normal Subjects

Purpose: Aim of this study was to evaluate the value of the photopic negative response of the blue-on-yellow-flash-electroretinogram in open-angle glaucoma. Viswanathan et al. (1) and Drasdo et al. (2) have shown a significant reduction of the photopic negative response in glaucoma using red flashes on a blue background or a blue-green silent-substitution-technique on a red backbround.
Methode: In 39 eyes of 39 normals (53±14 years, MD 1.34±1.3 dB) and 39 eyes of 39 patients with open-angle glaucoma of the Erlangen-glaucoma-registry (59±10 years, MD 11.33±6.4 dB) ganzfeld flash-electroretinograms (LKC, UTAS 3000) with blue Xenon-flashes (440 nm) of increasing photopic luminance on a bright yellow background (550 nm) were measured. Age-matched amplitudes and implicit times of a-wave, S-cone and L&M-cone b-wave and of the photopic negative response were compared between glaucomas and normals (unpaired t-test).
Results: The amplitude of the photopic negative response of the blue-on-yellow-flash-electroretinogram was significantly reduced in open-angle glaucoma for all flash intensities (p<0.001). Implicit times of S-cone and L&M-cone b-wave were significantly longer in glaucomas (p<0.05). The other parameters differ not significantly.
Conclusions: The blue-on-yellow-flash-electroretinogram might be a useful tool in glaucoma diagnostics for testing the blue sensitive pathway of retinal function. (SFB 539)
