Excimer-Laser Trabeculotomy from Interno - First Results

This video demontrates the ablation of trabecular meshwork ab interno with Excimer Laser using an Osher gonioscope. Every to some extent experienced anterio segment surgeon can apply this method safely after a short training phase. During an Excimer Laser trabeculotomy ab interno a 0,9mm limbal incision in the upper temporal quadrant is made, the anterior chamber is filled with viscoelastic, an optic fibre with obliqued top is inserted on the opposite side of the anterior chamber and applied on the trabucular meshwork. A sequence of Excimer Laser pulses opens a passage to the Schlemm canal. Each treatment consisted of 10 Excimer Laser apllications on the inferior nasal quadrant. With a very short training, the Excimer Laser trabeculotomy enables the surgeon to perform a minimal invasive operation with, compared to other surgical techniques, bares significantly lower risks, shows good results and does not obstruct possibly necessary additional measures. This method will make a contribution to diminish the shyness os surgical glaucoma treatment which is typical for German-speaking countries.
