The Influence of Topical Mydriasis with Tropicamid, Phenylephrine and Cyclopentolat on Aberrometric Measurements and Pupil-size. A Comparison

Purpose: To assess the influence of three different mydriatic drugs on the aberrometric measurement and pupil size before LASIK surgery.
Method: Prospective 9 eyes of 7 persons with emmetropia received topical dilatation of the pupil with Tropicamid 1% (Mydriatikum Stulln®), Phenylephrine 5% (Neosynephrin-POS®) and Cyclopentolat 1% (Zyklolat®) eye drops with 1 week break in between. After the application of the drops the eyes were measured with the Zyoptics aberrometer (Bausch & Lomb Surgical Corp.) every minute for 30 minutes then every 10 minutes till 1 hour. The results were compared with the subjective and objective (Autorefractor, Nidek) refractive error in miosis and aberrometric measurements without eye drops.
Results: The refractive error was subjevtively 0.61±0.26D and objectively 0.35±0.3D. The aberrometric measurements without any mydriatic drugs were 0.66±0.25D. After Phenylephrine the aberrometric measurements ranged between 0.74 and 0.81D, after Tropicamid between 1.09 and 1.16 and after Cyclopentolat between 1.17 and 1.26D. Similar largest pupil sizes (range 7.33 to 7.41mm) were measured after 40 minutes in the Tropicamid and Cyclopentolat group and after 60 minutes in the Phenylephrine group.
Conclusions: The three eye drops lead to hyperopic results in aberrometric measurements, the difference to the subjective refraction is smallest in the Phenylephrine group, but this group achieve the largest pupil sizes delayed.
