The Comparison of Different Reference Planes Acquired by Regional Analysis of Retinal Heigth in Patients with Macular Hole by means of Heidelberg Retina Tomograph

Objective: The height of the reference plane is crucial for many parameters of the Heidelberg Retina Tomograph (HRT). If the conturline (CL) dependend reference plane does not provide satisfying results the user is able to define its own reference plane. We investigated which retinal area correlates best with the heigth of the retinal pigment epithelium (RPE) in patients with macular hole.
Methods: We examined 22 eyes with macular holes of at least stadium II by a 20°-HRT scan and measured the mean hight of the following areas: 1) bottom of macular hole (=RPE), 2) papillomacular bundle, 3) 90°-sector temporal of the optic nerve head and 4) reference plane with correct CL, 5) enlarged CL and 6) too small CL.
Results: Compared to the RPE we found the following heights: papillomacular bundle + 65 ± 123 mm and 90°-sector + 150 ± 360 mm above the RPE; reference plane with correct CL - 103 ± 201 mm, with enlarged CL - 81 ± 191 mm and with too small CL - 126 ± 253 mm below the RPE.
Discussion: The height of the papillomacular bundle showed the best correlation to the RPE and could assist in the search for an alternative reference plane in 15°-HRT scans. The conturline dependend reference plane was more unstable especially in case of a too small contur line.
