Cyklophoto-versus Cyclocryocoagulation for the Treatment of Secondary Glaucoma

Purpose: Cyclophotocoagulation (CPC) is a recent alternative to cyclocryotherapy (CC) to decrease the intraocular pressure (IOP) in patients with secondary glaucoma.
Method: In this randomized prospective study 26 patients were treated by CPC (n=14) or CC (n=12). Changes in IOP, postoperative pain assessed with the visual analague scale (VAS), glaucomatous therapy and complications were recorded.
Results: In the CPC-group the mean preoperative IOP was 36,81±16,75 mmHg and in the CC-group 42,87±6,41 mmHg. Postoperatively the IOP decreased after 6 weeks to 18,5±5,02 mmHg (p=0,024) in the CPC-group and to 21,43±11,63 mmHg (p=0,018) in the CC-group. These values remained stable after 6 months with no significant differences between both groups. The mean postoperative pain values (VAS) were 4,705±4 in the CPC-group and 5,29±2,25 in the CC-group. In the CPC-group there were no complications, whereas in the CC-group two patients were retreated. Five patients required additional panretinal photocoagulation. No patient developed hypotonia.
Conclusions: The preliminary results of this ongoing randomized prospective study revealed that CPC and CC are equally effective in decreasing IOP by 50% in patients with secondary glaucoma.
