Using of Erbium:YAG-Laser in Modern Eyelid Surgery

Background: The new methods of laser treatment became very important also in modern eyelid-surgery. Especially the Erbium:YAG-Laser is used alternatively to conventional lid-surgery more and more in case of periocular affections of skin. Material and methods: We worked with the Erbium:YAG-Laser of Asclepion-Meditec Inc., MCL 29 Dermablate (wavelength: 2940 nm, pulse energy: 100-2000 mJ, durance of pulse: 250µs, rate of repetition: single pulse - max. 28 Hz, spot size: 1, 3, 5 mm). The study includes the treatment of 63 patients suffering different skin changes like xanthelasmata, tumors, seborrhoic keratosis, lentigines, scares, also skin resurfacing (treatment of wrinkles). In opposite of these above mentioned persons we report of 63 patients, who underwent conventional surgical methods. We compare preparation, going-on, postoperative treatment, progress in healing and cosmetic results of surgery. Results: The Erbium:YAG-Laser efforts a reduced preparation of the surgery. His handling is easy and save. It shows as a non-contact procedure with nearly no thermo effect healing with rare complications and very good cosmetic results in plane skin affections, which concern the surface of skin. We observed nearly no side effects like post operative scars or pigment changes, if the dermablation was only in the epidermis. The laser is very limited in treatment of deep skin affections. Conclusions: The dermablation by Erbium:YAG-Laser completes the methods of the conventional lid surgery and other laser methods. Especially the careful treatment of skin tissue and the rare incidence of bleeding in correct use of the laser is to be emphasized.
