The Optic Lamellar-perforating Keratoplasty with Stemcell Transplantation in Cases of High-risk Situations

Purpose: For stemmcellininsufficiency and high risk cases a maximum amount of stemmcellpopulations should be transplanted. This hinders the optical rehabilitation. A shortly new developed combined optical - lamellar and perforating procedure is evaluated for the optimal compromise of the two principles.
Method: Four eyes of four patients with stemmcell insufficiency due to conjunctival pemphigoid and metaherpetic keratitis a rectangular full transplantation was inserted in a lamellar bed. The long side was 15 mm, the small side 9 mm. The 7 mm central perforation of the recipient cornea was also performend.
Results: In a mean follow up of 1,2 years in all cases the transplant could be kept clear. In two cases secondary glaucoma occurred but could be regulated. For better immunological tolerance local and systemic immunsupressive substances were given.
Conclusions: The described surgiral procedure is newly developed. The combination of lamellar and centrally-perforating keratoplasty is possible. By the peritomy-like covering of the stemmcelltransplant by the conjunctiva and tenon and in vivo culture with serum components can be imitated. By contact of the transplant to the anterior chamber the immunprivilege can be involved. The conditioning of the mucosa of nose and throat let to tendencially better immunological acceptance.
