Photodynamic Therapy (PDT) of Classic Choroidal Neovascularization (CNV) in Membrane-proliferative Glomerulonephritis Type II (GN)

Purpose: PDT in age related macular degeneration (AMD) is a well established therapeutic concept. Membranoproliferative GN type II leads to a ‘pseudo’-AMD in the eye following a cnv as a late complication.
Method: We report on a 31 year old male patient suffering from membranoproliferative GN II since the age of 8. Renal histology shows a medium severe membranoproliferative GN with glomerulous and tubular scars. Funduscopy of both eyes reveals large areas of soft drusen and a 100% classic cnv (OS). The left fundus shows a greybrownish subretinal membrane, hard exsudates and soft drusen. The visual acuity is 20/20 on the right eye and 4/20 on the left eye. Visual acuity improves to 5/20 after PDT. Fluorescein angiographic findings show a diminished leakage and reduced hard exsudates. A second PDT follows due to residual leakage.
Results: Membranoproliferative GN type II leads to ‚pseudo’-AMD at the posterior eye. In case of classic cnv PDT is as effective as in AMD.
Discussion: In case of GN II associated ‚pseudo’-AMD with classic cnv PDT was performed successfully. The genesis of cnv does not seem to influence the outcome of PDT.
