Experimental Evaluation of Optical Online Pachymetry

Purpose: Optical online pachymetry allows to monitor changes of the central corneal cross-section intraoperatively. In this experimental evaluation the validity of the optical measurements was assessed.
Method: Optical online pachymetry (4Optics AG) with a wavelength of 1310 nm and a measurement frequency of 74 Hz was directly integrated in a clinical excimer laser (Esiris, Schwind). In 6 patients the central corneal thickness was measured with ultrasound pachymetry (US) and optical online pachymetry (OP). Furthermore, the ablation characteristics were assessed on not transplantable corneal buttons and synthetic samples.
Results: Optical online pachymetry was possible in all patients and materials studied with a good agreement compared to ultrasound pachymetry. The mean central corneal thickness was 529±44 µm (OP) and 539±33 µm (US). The reproducibility was ±7.7 µm (1.46%) with OP and ±5.0 µm (0.93%) with US. There was a significant linear relationship (r = 0,99; p < 0,05) between the calculated and the optically determined ablation depth.
Conclusions: In this experimental evaluation optical online pachymetry revealed to be a precise and reproducible method to monitor intraoperatively corneal thickness changes. This could be important for refactive procedures of the cornea, and particularly for PRK, LASEK and LASIK.
