Long-term Results after Excision of Periocular Basal Cell Carcinoma

Purpose: To evaluate data for patients with basal cell carcinoma in the eyelid region and to prove the efficacy of the treatment on basis of long follow-up period.
Methods: Retrospective analyses of 347 basal cell carcinomas treated by surgical extirpation and microscopic control of wound margins followed by reconstruction of the defects. The data were compared according to the age and gander of patients, localisation and size of tumours and number of recurrences of basal cell carcinoma.
Results: The carcinoma was mostly found in the patients 50 to 60 years of age, more often in women. The largest incidence of carcinoma was on the lower eyelid, near the edge or affecting it. The majority lesions were 5 - 15 diameters in size. The follow-up period of all analysed patients was longer than 5 years (5 to 20 years). The recurrence rates were observed depending on weather the margins were free or involved on histologic examination of the excised specimens. The overall recurrence rate was 8,1%. In the cases with histologically complete excision the recurrence rate was 2,6%.
Conclusions: The greatest risk of recurrence exist for basal cell carcinoma of the medial canthus. The surgical excision with microscopic control of wound margins has good results with low rate of recurrence even in long folow-up period. Microscopic control of wound margins after surgical excision allows low rate of recurrences and correct diagnosis in early stage permits good functional and cosmetic results.
