A New Device in Detecting Single-sided Aggravation of Blurred Vision

Background: Persons confronted with slightly different pictures on both eyes will usually not be able to know which of the pictures is seen by which eye. They may, however, learn it by closing one of their eyes. The aim is to achieve that the patient cannot differentiate between the impression of the left and right eye. Consequently, he should not be able to simulate or aggravate blurred vision on one eye.
Methods: A patient is confronted to optotypes projected in differently polarized light. Using two polar filters each eye perceives the correspondent optotypes. A lever-switch mechanism on each eye lid ensures that the optotypes disappear if the patient closes one eye. Therefore he should be unable to decide which eye sees which optotypes.
Results: The device described above was tested on persons with good visual acuity on both eyes. They were not aware which optotypes corresponded to which eye. Having blurred their vision with a +0.5 dpt. lens on one eye, they could not differentiate which side was blurred. Binocular vision is essential in order to use our device successfully. Heterophoria must be corrected by prisms.
Conclusions: The system described above is an appropriate device to detect single-sided aggravation of blurred vision reliably. Testing it on a group of aggravating patients will be subject of further investigation.
