Autologous Stem Cell Transplantation with Pedicle Flap of Conjunctiva for Prophylaxis against Relapses in Pterygiumexcision – A Clinical Study

Purpose: After pterygiumexcision up to 30% of the operated patients have relapses. In order to reduce the frequency of the relapses a new technique of surgery is used.
Method: In 37 patients (male:female - 23:14) an autologous stem cell transplantation with two pedicle flap of conjunctiva was performed. With the transplant the removed area of limbus and sclera should be completely covered. The mean of the excisions breadth was 8.2 mm. Post surgery patients got antibiotic eye-salve for one week and local applications of steroids for 4 weeks. The average time of follow-up was 9 month, minimum 6 month.
Results: 5.4 % of the operated patients (two patients) developed relapses. When discharged the visual acuity was on average about one line of the optotypes below the state before surgery. The spheric equivalent had increased about 0.2 diopters. In 4 cases the astigmatism increased between 0.25 and 2.0 diopters, in 7 patients the astigmatism could be reduced between 0.5 - 1.0 diopters. At the end of the follow-up the preoperative visual acuity and the refraction values were the same as before the surgery. Complications such as necrosis of sclera, ulcers or persistent erosiones were not found.
Conclusions: The coverage of defects because of pterygiumexcision with two pedicle flap of conjunctiva is a method with low risk of complications and a low rate of relapses.
