Intraoperative Two-dimensional Optic Coherence Tomography of the Anterior Segment

Background: Optical coherence tomography (OCT) is a new diagnostic tool that can be adapted to a slitlamp microscope to examine anterior segment structures with a higher resolution than ultrasound biomicroscopy. We examined the viability of adapting an OCT to a regular operating microscope.
Material and methods: A 1310 nm OCT was coupled via an external mirror, that was transmissible in the spectral range of visible light, to the outside of operating microscope optics. It was used in a non-contact-mode during a variety of anterior segment procedures.
Results: By means of OCT various surgical manipulations on the ocular surface as well as glaucoma procedures could be documented. Useful information on the depth of lamellar dissections, e. g. during deep anterior lamellar keratoplasty were could be obtained online.
Conclusions: Optical coherence tomography can be used for intraoperative diagnostic purposes and documentation of anterior segment procedures. This can be of particular use in lamellar dissections.
