Modern Astigmatic Surgery

The correction of astigmatism might be still a problem for the anterior segment surgeon. This surgery is used in refractive cataract surgery, in corneal refractive surgery and in the correction of keratoplasty astigmatism. In catarct surgery we can reduce astigmatism by choosing different incisions in temporal, oblique or superior position with corneal oder corneoscleral access. A combination with limbal keratotomies with different depth, diameter and seize is possible and leads to a reduction of up to 2 Dpt. Higher astigmatism up to 10 Dpt can only be reduced with arcuate or transverse keratotomies with different seize, diameter and depth. The correction of transplant astigmatism is possible with keratotomies or in a higher amount up to 20 Dpt with wedge resctions and sutures. The different technics and complications of astigmatic surgery are presented.
