Possibilities of the Waterjet in the Ophthalmology Wilhelm F.,
The waterjet is in the industry a well established device. In different surgical procedures (liver, brain etc.) it used already routinely. In the ophthalmology was already reported about its use in the refractive surgery. The stromal cut was done using a waterjet microkeratom and the epithelial debridement before using the excimer laser too. The mostly performed surgical procedure in the ophthamology is the cataract extraction. It is done today on a very high level. But there are still problems. The endothelial cell loss during and after the procedure and the secondary cataract are two of them. During the cataract surgery a lot of fluid is passing the anterior chamber of the eye. Why should this fluid not been used to emulsificate the nucleus of the lens and to polish the lens capsula to prevent secondary cataract? It was shown, that hard human lens nucleus could be emulsificated by the waterjet. By the changing of the corneal endothelium and the lens capsula of enucleated pig eyes the parameters of a waterjet device for ophthalmic surgery ware determined. It is necessary to create special hand pieces for reaching a high efficiency in emulsificaion the lens nucleus of the waterjet without damaging the lens capsula. Additionally the polishing of the epithelial cells from the lens capsula to prevent development of secondary cataract is more effective than conventional methods. On the lens capsula of enucleated pig eyes a significant lower number of epithelial cells an cortex especially in the equatorial region are visible after using the waterjet than after manual polishing. Other establishing of the waterjet in ophthalmic surgery are could be the endoscopic surgery of the lacrimal tubes and the retinal surgery via pars plana. The exact dosage and the possibility to manipulate very carefully without any increasing of the temperature especially in endoscopic surgery news is opening new possibilities for using the waterjet. |
