Therapy of Normal Tension Glaucoma: Effects of Latanoprost and Bimatoprost on Ocular Hemodynamics

Purpose: Altered ocular perfusion is discussed as an important pathogenetic co-factor in primary open angle glaucoma and particular in normal tension glaucoma. The locally applied prostaglandine analogues bimatoprost and latanoprost are very effective for reduction of intraocular pressure (IOP). Little is known about the hemodynamic effects of these therapeutics. Method: In a prospective, randomized and for the investigator blinded trial the effects of bimatoprost (Lumigan®) and latanoprost (Xalatan®) on intraocular pressure and ocular hemodynamics were studied. Therefore, the right eye of 21 patients with normal tension glaucoma were subjected to coulor Doppler imaging (CDI) shortly before and after a 3-5 weeks therapy with bimatoprost and latanoprost, respectively. For CDI a Siemens Elegra system with a 7.5L40 transducer was used. Results: 10 patients were treated with latanoprost and 11 patients with bimatoprost. There was no significant difference in all measured quantities and glaucoma defining parameters between the both therapy groups prior to therapy. IOP was reduced by 3.2±1.1mmHg in presence of bimatoprost and by 2.8±1.5mmHg in presence of latanoprost (bimatoprost vs. latanoprost: n.s.).
