Tear-off of the Optic Nerve after a Serious Blunt Trauma

Purpose: Case report of a 36-year-old patient suffering from a serious blunt trauma by an umbrella.
Method: Additionally to the usual clinical examinations ultrasound and CT examinations were carried out initially and repeated in conjunction with photographic documentation in the postoperative course. A pp vitrectomy was carried out because of an assumed retinal detachment combined with a vitreous beeding and missing light perception.
Results: A tear-off of the retina from the optic nerve was found intraoperativly. A silicone oil tamponade was added because of persistent hemorrhage from central retinal vessels. The imaging methods provided no hint towards a lesion of the optic nerve. In the later course a retraction of the optic nerve could be demonstrated.
Conclusions: A heavy blunt trauma to the globe caused the retina to be torn off the optic nerve. Inital imaging did not show this complication. The mechanism of this injury indicates the force meeting the optic nerve in an orthograd direction.
