Femtosecond Laser LASIK Flap Preparation with Conical Incision and Positional Pikes

Purpose: The purpose of the present study was to describe the possibility to form a corneal flap for laser assisted in situ keratectomy (LASIK) with sagittal or conical incisions and with positional pikes which may increase the postoperative rotational stability of the flap. Method: A corneal flap was prepared in five freshly enucleated porcine eyes, using a femtosecond laser. In a first step, an intrastromal incision running parallel to the corneal surface was created with a diameter of 8 mm. In a second step, an posterior-anterior incision was carried out with a diameter of 8 mm at the level of the first incision and a diameter of 7 mm at the corneal surface. Additionally, a hinge and three pikes were prepared into the flap with corresponding notches in the surrounding tissue of the flap. Results: For all eyes, the corneal flaps could be prepared without major difficulties within less than 5 minutes. Conclusions: Femtosecond laser technology allows a new type of LASIK flap formation with conical incisions and positional pikes.
