Conjunctiva Prophylaxis with a Combined Topical 0.3% Ofloxacin and 5% Iodine Irrigation is Ten-fold More Effective in Reduction of the Bacterial Normal Flora before Cataract Surgery

Purpose: To determine the effectiveness of a prophylactic regimen for conjunctival bacterial load reduction prior to cataract surgery.
Method: This prospective study included the application of topical 0.3% ofloxacin (4 times/day for 3 days and 3 times 1 hour prior to surgery) and povidone iodine 5% (PVI) with 45 patients, as their periorbital area was scrubbed and their conjunctival fornices were irrigated with 10 ml solution, 5 minutes prior surgery. Conjunctival samples originate from T0 (baseline, 5 days prior to surgery), T1 (after 3 days of topical ofloxacin), T2 (immediately before surgery) and T3 (at the conclusion of surgery), whereas anterior chamber samples only stem from T2 and T3.
Results: T0 yielded 42/45 positive cultures (93%) with 96 % Gram-positive bacteria. At T1 the bacteria load was reduced 24% to 32/45. At T2 4/45, at T3 2/45 indicated positive growth, that is a reduction of >90% (P<0.001). All anterior chamber aqueous samples (T2/T3) were sterile.
