Sudden Unilateral Loss of Vision due to a Carotic-Ophtalmic-Aneurysm

A 43-year-old-man presented a sudden, painless unilateral drop of vision (LE 4/10; RE 10/10) without traumatic incident. The visual field showed a diffuse reduction detectable in all quadrants and an elargement of the blind spot on LE. A magnetic Resonance imaging (MRI) examination of the orbita and the brain demonstrated a large (12 X 14mm) paraclonidal carotic-ophtalmic aneurysm with compression of the left ocular nerve. The patient was immeditealy transfered for operation to the department of Neurosurgery at the University of Bern (Switzerland) where a clipping (2 Yasargil-Standart-Clips) of the aneurysm by osteoplastic craniotomy was performed. The patient recovered uneventfully, vision one year after the operation is 10/10 on both eyes with no defect on visual field detectable.
