Vitreous Serum Albumin as a Cause of Precipitate Formation and Emulsification under Vitreous Tamponade with Hydrofluorocarbons

Purpose: To investigate the role of precipitate formation and emulsification after ocular endotamponade with hydrofluorocarbons (HFCL). Method: A) Intraoperative specimen of precipitates under HFCL-tamponade were analyzed by electrophoresis and MALDI-TOF analysis. Protease-digested peptide fragments have been identified by N-terminal Edman-sequence-analysis. B) Human serumalbumin (HSA) and different vitreous endotamponades (O62, O62-Oligomer, F6H8, PFD, silicone oil) were rotated in transparent vials. Results: A) Chemical analysis of the "in vivo" precipitate containing sample detected modified HSA. B) Precipitate formation was only observed mixing HSA with O62-Oligomer. Dispersion/emulsification alone was observed in O62-Oligomer > O62 > F6H8 > Silicone oil. The size and transparency of droplets differed markedly. Conclusions: The increased amount of HSA in PVR-cases with severe blood aqueous barrier breakdown can precipitate in presence of certain fluorocarbons and cause emulsifications of HFCL in variable extent. HSA may be modified by fluorocarbons and act as an antigen causing severe inflammatory
