Reconstruction of the Anterior Chamber in Progressive Congenital Iris Cyst with Subsequent Corneal Decompensation

Purpose: Iris cysts are rare congenital lesions of unknown etiology and classified in pigmented and non-pigmented cysts. The therapy of choice consists in a wide en-bloc excision of the cyst and the surrounding tissue with cornea, sclera and ciliary body to minimize the risk of local relapse and diffuse epithelial invasion. We demonstrate the successfull therapy of a cyst with the combination of sector iridectomy, penetrating keratoplasty and implantation of a posterior chamber lens in a 50 year old male patient. He presented with visual loss to hand movements in addition to corneal as well as tensio decompensation on his better right eye. Ocular examination revealed an iris cyst within the right eye in contact with the corneal endothel and covering the pupil. The iris cyst had been diagnosed in early childhood. Slit-lamp findings of the left eye were broad frontal synechias. The visual impairment was 1/15 since early childhood. Additionally our patient suffered from cataract.
Method: After stabilizing the intraocular tension the cornea was excised and the iris cyst via open-sky technique in toto removed. Therefore, a partial resection of iris tissue was performed before the cataract was removed and the IOL and a corneal graft was implanted.
