Thrombophilic Risk Factors in Anterior Ischemic Optic Neuropathy

Purpose: Over the past years, several studies have strengthened the hypothesis that coagulation disorders are pathogenic for retinal vessel occlusion. However, thus far, the question whether thrombophilic disorders are associated with the development of nonarteriitic anterior ischemic optic neuropathy (NAION) remains a matter of controversy.
Method: In our prospective study, a cohort of 18 unselected NAION patients (< 60 years of age) was screened for thrombophilic disorders and compared to a control group of 30 healthy subjects. Statistical analysis of differences between corresponding groups was performed by Fisher´s exact test.
Results: Overall, thrombophilic disorders were found to be present in 6 of 18 patients but only in 2 of 30 controls (P = 0.039). 2 of 18 patients and none of 30 controls were tested positive for homozygous resictance to APC. Moreover, a previous thromboembolic event in the absence of cardiovascular risc factors was significantly associated with a high prevalence of coagulopathies (5 of 6; P = 0.0039).
Conclusions: Our results indicate that thrombophilic risc factors are highly prevalent in NAION patients < 60 years of age and/or a history of thromboembolism at this age.
