Comparison of Retinal Nerve Fiber Layer (RNFL) Measurements with Optical Coherence Tomography (OCT) and Retinal Thickness Analyser (RTA)

Purpose: Measurement of the RNFL can be of importance in detecting progressive glaucomatous damage. Several machines are well suited for measurements of the RNFL (Nerve Fiber Analyser (GdX), Heidelberg Retina Tomograph (HRT), OCT and RTA) and are commercially available. In this study we compared topography images from the RTA and the OCT as topography images can show defects at a glance and are superior for the clinically trained examiner.
Method: 20 eyes of 20 patients with ocular hypertension or glaucoma were included in this preliminary analysis. We performed RTA (RTA II, glaucoma analysis and retinal thickness map) and OCT (OCT II, concentric rings and RNFL map) examinations and compared the false colour coded images of the peripapillary RNFL.
Results: 4 patients showed a good correlation between the two methods, 10 patients showed moderate correlation and 6 patients showed no correlation
Conclusions: One reason for the unsatisfactory correlation of the two methods was a suboptimal centering algorithm by the RTA. Another reason was that the OCT shows sectorial analysis while the RTA results must be calculated from selected values across the posterior pole. A revision of the inbuilt data analysis of the two machines could yield a more comparable form of RNFL images.
