ICCE and Scleral Suture-fixated IOL via Tunnel Incision

Purpose: Even today, certain ocular situations require intracapsular cataract extraction, particularly when extensive zonular dehiscence is present. Historically, cryoextraction was applied for the intracapsular procedure, which required a rather large incision size to the anterior chamber.
Method: In the presented video, we demonstrate a series of surgeries, where intracapsular extraction without cryo was performed via tunnel incisions of varying size. In most cases, the procedure was combined with scleral suture-fixation of a posterior chamber lens. In young patients with a rather soft lens, incision size did not need to exceed the diameter of the IOL optic.
Conclusions: The presented intracapsular extraction technique is far less traumatic compared to cryoextraction. It is an elegant technique especially in young patients, where incision size does not need to be much larger than for phacoemulsification.
