Penetrating Keratoplasty in Eyes with Buphthalmos

Purpose: Purpose of the study was to evaluate the prognosis and complications of penetrating keratoplasty (PK) for corneal decompensation in eyes with buphthalmos and to analyze the risk factors for graft failure.
Method: Clinical records of 13 adult and 3 pediatric patients who underwent PK for endothelial decompensation with a diagnosis of congenital glaucoma were reviewed retrospectively. During the study period between January 1987 and December 2001, 33 PKs were performed in 20 eyes with buphthalmos out of a total of 3663 PKs performed in our department (0,9%). The median age of the patients at the time of PK was 39 (3 - 72) years. All patients had a history of previous intraocular surgery, including glaucoma surgery, cataract extraction and PK. The impact of pre-, intra-, and postoperative factors on graft failure and duration of graft clarity was analysed.
Results: 55 % of the eyes (11/20) received one graft, 25% (5/20) two and 20% (4/20) received three grafts. During a mean follow-up of 87 (4,5 - 172) months, graft failure occurred in 18 of 33 grafts (54%). 7 eyes (39%) had immunologic, 11 eyes (61%) had nonimmunologic graft failure.
