Early or Late Infantile Strabismus Surgery Study. A non-randomised, prospective, multi-centre clinical trial
Early vs. Late Infantile Strabismus Surgery Study-Group
Purpose: The ELISSS is a European group of strabismologists and orthoptists that investigated whether early or late surgery is preferable in infantile strabismus in a prospective, multi-centre trial. Infants age 6 - 18 months received a standardized entry examination and were operated either before age 24 months or between age 32 - 60 months. All children were evaluated at age 6. The two groups were compared regarding degree of binocular vision, angle of strabismus and relative visual acuity of the worse eye.
Method: Mean age at first surgery was 20 and 49 months, respectively. 405 patients completed the study, 171 early and 234 late ones. For a study duration of 10 years the dropout rate of about 24% is low. There are no differences in dropout rates between both groups.
Results: Early surgery leads to better stereopsis and higher amount of simultaneous perception than late surgery (p=0.001). This seems to be mainly due to fewer patients in the early treatment group who have no binocular vision at all (32.4% Bagolini negative in the early treatment group, 47.4% in the late one). Only 1% of both groups had positive stereopsis using randomised tests. The classified angle of squint at the final examination does not differ between the two treatment groups (p=0.97).
