Excision of an Atypical Melanocytic iris Tumour and Later Optical Rehabilitation by Cataract Surgery and Implantation of a Coloboma Capsular Tension Ring

Purpose: We report about a 61 years old female patient with iris tumour, which showed a significant increase in size during the last 15 years documented by photographs. We want to discuss the possibilities of surgical intervention in this case.
Method: The atypical melanocytic iris lesion was excised. The material was examined pathohistologically and a spindle cell nevus of borderline type (group 6 Zimmermann) was found. Post surgery the patient suffered from progressively impaired vision and increased sensitivity to light caused by clouding of the lens and iris coloboma. No additional complications occurred. Therefore four months after tumour excision cataract surgery had to be done. In a second step a coloboma capsular tension ring had to be implanted for covering the iris coloboma caused by primary surgery.
Results: Six weeks after secondary correction the patient did not have any reduction of vision and did not suffer from glare anymore.
Conclusions: This case demonstrates that excision of an iris tumour can be performed without any lasting handicaps, if it is followed by optical rehabilitation afterwards.
