Purpose: Accurate axial length measurement by ultrasound can be complicated, if the eye is filled with silicone oil after vitreo-retinal surgery. Partial coherence interferometry (PCI) by the IOL-Master (Zeiss) may be a valuable clinical alternative. Method: Axial length measurements with the IOL-Master were performed in 45 eyes with silicone oil endotamponade following complicated vitreo-retinal surgery. At least two measurements with a signal-to-noise-ratio (SNR) of 2.0 or larger should be achieved. We investigated the number of cases in that PCI were not feasible, we compared the SNR of silicone oil-filled and contralateral eyes without endotamponade. The intended postoperative refraction could be compared with the actual refraction after combined oil removal and cataract surgery in 6 eyes. Results: Owing to dense cataract, retinal detachment or lack of fixation PCI achieved no reliable axial length measurements in 4 out of 45 eyes. The mean SNR of two PCI measurements exceeded 4. Performing more than 2 measurements the mean SNR exceeded 5 and approached to the SNR of the control eyes. Postoperative refraction was within a range of 1.5 dpt of the intended refraction in 5 out of 6 eye in that combined oil removal and cataract surgery had been performed. Conclusions: Even in eyes with retinal pathology and silicone oil endotampona
