Vitrectomy for Traumatic Macular Lesions in Children

Purpose: Vitrectomy for traumatic macular lesions is rarely performed in children. In this study, we report on our experience and results of vitreoretinal surgery for treatment of traumatic macular lesions such as traumatic macular hole and premacular hemorrhagic cyst in eyes with Terson's syndrome and shaken baby syndrome.
Method: Between November 1995 and October 2002 10 eyes of 9 children underwent macular surgery for macular lesions. Patients age ranged from 7 months to 17 years. Indication for vitrectomy were traumatic macular hole (4), premacular hemorrhagic cyst in eyes with Terson's syndrome (4) and shaken baby syndrome (2). During vitrectomy rhexis of internal limiting membrane was performed. 4 children received intensive orthoptic treatment.
Results: Improvement of anatomic situation and visual acuity was achieved in the majority of eyes (8). Follow-up ranged from 6 months to 5 years.
Conclusions: If macular surgery in pediatric eyes is indicated, it should be performed as soon as possible to avoid amblyopia. Special conditions of pediatric eyes must be taken into consideration.
