Regeneration of the Myogenic Response in Retinal Arterioles of Diabetics

Purpose: A non-invasive in vivo-measurement of the myogenic response of retinal arterioles ("Bayliss-effect") is possible by the use of the Retinal Vessel Analyzer (RVA, IMEDOS Weimar). The purpose of this prospective study was to measure the regeneration of the myogenic response in diabetics when metabolic control is improved. Method: Patients with diabetes undergo an educational program at the Dept of Medicine II of the Friedrich-Schiller University, Jena to improve metabolic control. At the very beginning of the program a standardized measurement of the myogenic response of an retinal arteriole under raised blood-pressure was performed by the use of the RVA. 12 months later patients with improved HbA1c-values were exposed to the same blood-pressure provocation and the myogenic response was measured again. Wilcoxon-test was used for statistical analysis. Results: 17 patients were included with significant improved HbA1c-values (on average 9.4% to 7.06%; p<0.001). Exercise resulted in the same blood-pressure rise (p=0.629) on both measurements. The myogenic response improved from -2.9% vasoconstriction to -5.0% vasoconstriction after 12 months with improved metabolic control (p<0.05). Conclusions:
