Purpose: To assess the correlation between capsulorhexis size and visual outcomes in patients implanted with the 1 CU accommodative IOL. Method: 25 eyes underwent phacoemulsification and implantation of the 1 CU accommodative IOL. Three months after surgery, far uncorrected and best corrected visual acuity was assessed. Distance corrected visual acuity for near was evaluated also. Retroilumination photographs were taken to assess capsulorhexis size, centration and amount of overlapping. The photographs were analyzed utilizing EPCO. Results: Mean age of the patients was 53.5 years (30 73). Preoperative far UCVA improved from 0,05 to 0,7, and BCVA from 0,3 to 0,94. Near distance corrected visual acuity was 0,5 (0,1 1) that improved to 0,9 near corrected. Mean CCC size (4,3 mm) and decentration (0,35 mm) did not influence visual outcomes. However, CCC overlapping (mean 0,35; 35%, range from 16 to 55%) was correlated with Near visual acuity. There was a positive correlation between smaller overlapping (less than 0.35) and better near visual acuity. Conclusions: Smaller capsulorhexis gave better near visual results. We propose that an overlapping between 0,25 and 0,35, which correlates with CCC size between 4,5 and 5 mm is id
