Purpose: The intraocular lens power calculation by ultrasound-A-scan-biometry is difficult in patients with high myopia for example owing to staphyloma. Purpose of this retrospective study was to show whether lens power calculation by optical biometry leads to better results. Method: The examinations were performed on 22 consecutive, myopic eyes with an axial length > 26.5 mm (19 patients, 63 ± 13 yrs. old, average preoperative refraction of 12.0 ± 4.4 dpts.), which underwent phacoemulsification and implantation of an intraocular lens after biometry with the Zeiss-IOL-Master® and calculation with the Haigis formula. The postoperative refractive error was determined by comparing the postoperative spherical equivalent with the predicted value. Results: Measurements pointed out an average axial length of 28.5 ± 1,9 mm. 19 eyes showed a postoperative spherical equivalent which differed less than 1 dpt. from the predicted value, in 13 cases the postoperative refractive error was even below ± 0.5 dpts. Conclusions: Intraocular lens power calculation with optical biometry seems to deliver reliable results for cataract surgery in patients with high myopia. Further studies have to prove our preliminary results.
