Excessive Pigment Dispersion with Seclusion of the Pupil Following ARTISAN Lens Implantation in a Hyperopic Patient

Purpose: Implantation of phakic intraocular lenses is performed worldwide for the correction of moderate to high refractive errors. In this study we report a case in which the implantation of a hyperopic phakic intraocular lens caused severe postoperative complications.
Method: A 52-year-old female patient with a refractive error of + 5.5 diopters in the right eye and +3.5 diopters in the left eye underwent implantation of an ARTISAN iris-claw phakic intraocular lens elsewhere. One year after surgery, the patient presented for the first time at the University Eye Clinic of Innsbruck because of visual impairment and pain in her left eye.
Results: Ophthalmic examination at the University Eye Clinic of Innsbruck revealed an excessive pigment dispersion with a tumor-like pigment formation, posterior synechiae at the nasal margin of the pupil as well as patches of pigmentation on the intraocular lens surface. The patients current therapy consisted of topical corticosteroids and anti-glaucoma eye drops. The right eye showed a moderate pigment dispersion. Uncorrected visual acuity was 0.7 in the right eye and 0.6 in the left eye. Only one year later the patient presented again at our clinic and slit lamp examination revealed a partial seclusion of the pupil in the left eye and a progressive pigment dispersion in the right eye.
