VISUAL RESULTS AFTER BILATERAL IMPLANTATION OF ASYMMETRICAL DIFFRACTIVE MULTIFOCAL INTRAOCULAR LENSES

The bilateral implantation of diffractive multifocal intraocular lenses (MIOLs) with an asymmetrical light distribution for the far and near focus is a new concept for restoration of pseudoaccommodation after cataract surgery. One eye receives a far dominant MIOL which has a light distribution of 70% for the far and 30% for the near focus, while the fellow receives a near dominant MIOL with an opposite light distribution. The idea is to improve far and near contrast sensitivity with no compromise of binocular vision.
Patients: Twenty-two patients (49-84 yrs.) have been bilaterally implanted at two centers. Monocular and binocular distance and near visual acuity (Nieden, Nd) as well as contrast visual acuity using the Regan contrast charts were tested at 5 to 44 days postoperatively. Binocular visual function was examined in all patients.
Results: Mean corrected distance visual acuity was 0,96 (+/- 0,15) with the distant dominant MIOL compared to 0,81 (+/- 0,14) with the near dominant MIOL. Mean binocular distance visual acuity was 0,98 (+/- 0,14). Eyes with a near dominant MIOL demonstrated a better mean near visual acuity (Nd 1,5) than eyes with a distant dominant MIOL (Nd 2,1). Ninety percent of the patients had a binocular near visual acuity of Nd 2 or better. Contrast acuity was improved in the dominant focus of either MIOL compared to the non-dominant focus in the fellow eye. All patients showed a normal binocular visual function.
Conclusion: The concept of bilateral implantation of diffractive MIOLs with an asymmetrical light distribution for the far and near focus shows an improved visual function in the dominant focus with a normal binocular visual function.
